Observation entry meta
Date, time, location (lat / lon / site name), telescope / binocular, dark-adaptation minutes, on-site altitude, ambient noise, light-pollution Bortle estimate, trip purpose.
Sky observation + sky sketch
Constellation, planets, deep-sky targets, satellite tracks, meteors / fireballs, plus a 380x200pt sketch frame for the dome, the eyepiece, or the naked-eye view.
Weather box
Sky cover (clear / few / scattered / broken / overcast), wind speed + direction, temperature, humidity, moon phase + illumination %, Antoniadi seeing (I-V), transparency (1-10).
Landscape sketch + wildlife / terrain notes
Horizon, foreground, cloud types, plus a wildlife and terrain log — species, tracks, sound, behavior, encounter distance. Separate sketch frame sized for the wider view.
Trail / track notes + sketch

Each entry occupies a "sky" page (date, time, location, equipment, constellation / planet / deep-sky / satellites / meteors logs, sky sketch frame, track notes + sketch frame) and a "landscape" page (same meta repeated, landscape observation rows, landscape sketch frame, weather box). Six pairs = roughly a half-year of weekly stargazing.

The observation half benefits from AcroForm fields you can type into in any PDF viewer. The homestead half works the same way, but is also designed to print cleanly as a paper checklist — checkboxes, item text, and notes column line up at the same width so a quarter-end paper copy prints as fast as a digital fill.
